Identity Verification Policy
To ensure safe, accurate, and compliant medical care, all patients are required to verify their identity before receiving telehealth services.
 
What We Require
Patients must upload a valid government-issued photo ID, such as:
 • Driver’s license
 • State-issued identification card
 • Passport
 
The name on the ID must match the name used during registration.
 
Why This Is Required
Identity verification helps:
 • Confirm patient identity for telehealth visits
 • Maintain accurate medical records
 • Support patient safety and continuity of care
 • Comply with applicable telehealth and licensing requirements
 
Privacy & Security
Uploaded identification is used only for identity verification and is stored securely in accordance with privacy and HIPAA standards.
VitaRegen Medical™️ does not use identification for marketing or non-clinical purposes.
 
When ID Is Collected
 • ID verification is required prior to the first appointment
 • Patients will be prompted to upload their ID during registration or intake
 
Important Notes
 • Care cannot be initiated until identity verification is completed
 • This requirement applies to all telehealth services
